NCH&C Neurodevelopment Service

The Norfolk Community Heath & Care NHS Trust (NCH&C) Neurodevelopmental Service (NDS) assesses children and young people up to the age of 18 for Autism Spectrum Disorder (ASD) and/or Attention Deficit Hyperactivity Disorder (ADHD).

New Digital Referral Form

The new digital NCH&C Neurodevelopmental Service Referral Form is accessed through the professional login on JustOneNorfolk. The information requested in the new digital referral form will support more efficient and effective assessment processes for parents/carers, children and young people.

Making a referral

To make a referral

  1.  Log into the professional section of JustOneNorfolk. If you don't already have one, create a professional login for JustOneNorfolk. Please see attached Just One Norfolk Professional Logins (PDF, 78 KB) guidance for further information.
  2.  Once logged in, click on "NDS Referral" label in the light blue bar towards the top of the page.
  3. You will be brought to the front page of the NCH&C Neurodevelopmental Service Referral Form.
  4. Follow the instructions to make a referral - there is one form for children under 5 years old and a different form for children and young people aged 5 years and over.

Things to note

Referrals must be accompanied by

  • Early Years Observation Questionnaire or Teacher Observation Questionnaire (TOQ) (depending on the age of the child)
  • Parent Observation Questionnaire (POQ)
  • If under 5, a standardised developmental assessment report, for example, a Schedule Of Growing Skills II (SOGS) or similar.
  • If over 16, Young Person Self Report
  • Referrals must be completed in one go and cannot be saved to be completed later. We advise all referrers to have gathered all the required evidence to support a referral prior to starting to complete the referral form. It takes approximately 30 minutes to complete a referral.  We plan in the future to build in the function for referrers to save and return to the form prior to submitting.
  • Referrers will not get an automated acknowledgement email. Again, this will be addressed in the future.

NDS Transformation Project

At NCH&C, we are reviewing our Neurodevelopmental Service and introducing some changes to ensure our service is operating both efficiently and effectively, and to improve waiting times for assessment. We are part of a system-wide Neurodiversity transformation project working with our system partners (Norfolk County Council, Norfolk & Waveney Integrated Care Board, Families, other health providers) which will offer a new way of supporting children, young people, and families as soon as they recognise that they may need some support. 

The new NCH&C Neurodevelopmental Service Referral Form is part of the NDS Transformation Project.
