What is transition support?
Transition support from Norfolk's School and Community Teams (SCT) is a structured programme designed to help children and young people (CYP) navigate key educational transitions with confidence and support.
How does it work?
SCT offers a tiered approach—universal, targeted, and enhanced—to support children at key transition points. The support is delivered by Education and Family Workers in collaboration with schools and families.
What are the principles? What are the benefits?
The programme is inclusive, proactive, and child centred. It aims to reduce anxiety, build resilience, and ensure a smooth transition by involving families, schools, and community teams. It supports emotional wellbeing and school readiness.
Who and how can this intervention benefit?
Children transitioning:
- From early years to reception (via PEEP Transition Groups)
- From infant/primary to junior school
- From Year 6 to Year 7 (secondary)
It benefits children with emerging SEND, those experiencing anxiety, or those identified as needing additional support.
How can this intervention be used? What needs can it support? What CYP may benefit the most?
It supports:
- Emotional regulation
- Social confidence
- School engagement
Children with SEND, social communication needs, or those who have experienced disrupted education may benefit most.
How can schools/settings support with transition?
Schools can:
- Host group sessions
- Collaborate with SCT to identify children in need
- Provide space for targeted interventions
- Engage in follow-up support during the autumn term.
How can families support with transition?
Families can:
- Attend PEEP Transition Groups
- Engage in home-based activities
- Communicate with SCT and schools about their child's needs
How can you help CYP understand, be ready for and get the most out of their transition support?
- Use visual supports and social stories
- Visit new settings in advance
- Encourage open conversations about change
- Reinforce routines and expectations

What information/support do we have in Norfolk?
- SCT Transition Offer includes:
- Universal: SCT presence at transition events
- Targeted: Group workshops (PEEP) for up to 10 CYP and their parents with emerging SEND
- Enhanced: 1:1 home-based support over summer with follow-through into autumn
