Guidance on completing the online form

Be aware you will need to complete the form in one go – you cannot save it and return to it later. You will need to complete the form within two hours of starting to avoid a timeout. The form will also timeout if left inactive for a long period of time, so best not to take a break in the middle of it.

The form will ask you to provide the following information, so ensure you have this ready to avoid difficulty in completing the form:

1. Pre-referral checklist confirming the actions school has taken to support the pupil’s health and SEND needs (a copy of this checklist can be downloaded and viewed via the Medical Needs Service webpage)
2. The pupil herringbone and health guidance/health care plan (to be uploaded)
3. Demographic information about the pupil
4. Contact details for the referring school and parent/carer
5. Information about the pupil’s health and SEND needs, including professionals involved in their care and support
6. Guidance on how to support their access to education
7. Confirmation of parental consent to the referral and consent for the Medical Needs Service to contact health professionals if required

You will need to complete all the fields on the form. If you miss one out on a page, you will see a message asking you to go back and fill it in.
