Key Stage Assessments Submission Templates - 2025

EYFSP & Phonics Excel templates

If required they should be completed by schools and returned to the Data Team via Perspective File Request: 

EYFSP Excel template (Excel doc, 31 KB)

Phonics Excel template (Excel doc, 10 KB)

When completing the Phonics template ensure the Pupil Phonics scores (out of 40) are added to column J "Check Mark Result Qualifier (NM)" and ignore column K "Check Outcome Result Qualifier (NY)" which should be left blank.
